Certain services such as private sick notes, private medical insurance forms and holiday cancellation forms, are not covered by the NHS and charges are made in line with the recommendations of the British Medical Association. Typical examples are listed below. We need seven working days for the completion of any forms, reports, letters or photocopies. Please note we do not sign passport applications forms.
You must see your GP before making an appointment with a Consultant. We can only sign claim forms if your GP has initiated the referral. There is no fee for a private referral, but a fee will be charged if you need your health insurance claim form signed.
We do not automatically convert private prescriptions to NHS ones and we only do so when we have referred to the consultant and have received a full report from them. A referral does not mean we are under any obligation to follow the consultant’s advice. In addition, the medication prescribed must meet the approval of the pharmaceutical advisory committee of the Haringey Clinical Commissioning Group.
